Maternity grant
A midwife gave me a sure start maternity grant form with my matb1 form but me and my mum realised that it’s already filled out with some other random persons details ?❤️
What do I do cause I soon need to fill out the form so I csn get my grant I’m 27 weeks + 4 days 🤰🏽

I never had to show a MAT B 1 form for the sure start grant. Only to my employer. Couldve been because I already had a joint claim out with UC ( rarely get any help but jad a claim for when I was on maternity with my first) I only applied for the one off £500 grant. Which was with my first. Any other documents had to be uploaded via their page.
I'd contact your midwife to get another one redone x
