Your GP shouldn't refuse it... Have you tried popping over to your community midwives or the hospital? Or even your GP surgery. They should give you one with no appointment.
I work 8-6 during the week so I can’t actually get in to see my GP, I’ve just been doing the online forms each week annoyingly. I’ll try and phone the hospital and see if I can come in the evening and get one x
Ring your maternity triage and explain the situation, your GP can issue one as well as the community midwife. The maternity triage department may not have any but antenatal Clinic should who see out of area. They are meant to do an examination and will want to see you and your notes to fill everything in. It's called a MATB1 form. You normally get one at 25 or 28 week midwife appointment. Your work don't need one to allow you maternity leave it just enables them to claim SMP back from the government.
I’ve phoned literally every single community hub/maternity unit and they all keep sending me different places and the ones who answer send me to someone else or i get sent to a voicemail it’s really draining as i’ve not been seen by a midwife since december and my work want my form today before i finish🙄🙄🙄
That's really rubbish! I'd honestly just go in person - it's a paper form they have to fill out in pen so they won't send you a scan anyway. I know it's a bother with work and all but I'm not sure there's a way around it... As for not being seen by anyone since December, that's honestly even worse - I'd put in a complaint via PALS!
@Paulina Honestly I’m just stressed out with all of it🙄 My GP messaged me saying they’ve contacted the midwife on my behalf, it’s crazy, with my first I was given my midwife’s personal phone number and it was so much simpler, no idea why all I get this time round is just her name🥲
You should be able to get it from the hospital if you've no appointment due just pop in and ask one of them for it any midwife can fill it out
