Milk bleb and blocked ducts, advice please!

Help needed. So much conflicting advice on NHS website and BF network.
I'm exclusively BF my 4 month old, around 5am noticed my boob had a lump and felt blocked, it had been a long sleep for baby so fed him as usual expecting it to soften.
Post feed still hard lump there, throughout the day have kept feeding as usual and taking ibuprofen and paracetamol as the lump is uncomfortable and a bit hot. Tried gentle massage in the shower and cold compress, nothing is relieving the lump.
Noticed this evening a milk bleb on the nipple of the breast with the lump, not sure how long it's been there.
What should I do? Some things Say warm compress others say cold, some say try to squeeze the blockage out...
The boob itself and the bleb are hurting now.
Thank you for reading :)

I had a blocked milk duct several times with my first boy, I found gentle massage while feeding eventually cleared it. If it’s not gone within a few days or starts to cause a lot of pain and get red contact your GP as it could be mastitis x

If massaging whilst feeding isn’t relieving it, then apply something warm, gentle massage and have a short pump.
Since having mastitis when my Son was about 6 weeks I learnt to recognise the signs and if I ever feel a clogged duct now I use my lansinoh gel pads, massage and pump, this always resolves it for me. If you start feeling feverish/cold or flu like symptoms and getting redness/streaks then it’s probably turned to mastitis and you’ll need antibiotics to clear it up but hopefully you can ease it before then!
These gel pads are a saviour and i really recommend getting some x

I had a bleb on my nipple back in March, I think lasted maybe a week or two, like the other ladies have said massage and keep breastfeeding, your little will help remove it, just don't pick at it or try to remove it yourself.

Dangle feeding/pumping has always worked for me whilst gently massaging the lump. Only pump if you know your setup is spot on for you though and actually empties you, if not then hand express and then dangle feed + massage.

I had a blocked duct and the breastfeeding support team told me only use cold compress as lymph nodes send extra fluid to the area as they think they’re fighting infection so massages up towards the armpit to encourage fluid to move back and cold compress to help it not be inflamed.
The only thing that helped with my milk bleb was warm compress only on the nipple before feeding and then loads of milk starting coming out of it!

Hello, just wondering what happened in the end? Did you managed to sort the blocked milk duct?
I’ve currently got a lump too and have had it for around 4 weeks now! Went to the docs and they said to massage and express with warm compress etc which didn’t help. Also took antibiotics which didn’t help. Had a scan yesterday at the hospital and they said it’s a built up collection of milk and couldn’t remove it with needle due to the thickness of the liquid so had to take a biopsy so now I have to wait for the results and keep massaging! Honestly am so worried and frustrated too!! Have no idea what to do!

How long did you have to do the massages and cold compress for it to unclog?

I did a warm compress on my actual nipple before every feed for a day and mine cleared. For the blocked duct someone recommended a lactation massager to me so I used that the second time I got a hard lump on my boob, not sure if that was the reason my boob stopped being so hard. My boob had more of a wedge type feeling, like the top of my boob was hard and that lasted 2 days. I also offered my little one lots and lots of extra feeds

thank you! There is hope that mine can go…praying! I’ve had it for about a month now and honestly am panicking on how to get rid of it!
