Vaccination exemption form

Hello my daughter will be starting school soon and I was wondering if anyone knew where I can get the Vaccination exemption form so she can start school. I need help! I’m having anxiety.

I am a school nurse and if there is a vaccine exemption either medical or religious we have a form that we provide to the parents and have it renewed yearly. Have you contacted the public school and asked if they have a vaccine exemption form?

No it’s Daycare. I didn’t know where to start. So I have to ask the school for it? I was thinking I had to download it from a site. Hopefully they have it.
