Sure start maternity grant

Hi everyone! I wasn’t eligible for the sure start grant whilst pregnant as my salary has only just dropped and am now getting some UC, but I wasn’t before. I have taken my son’s birth certificate into the Jobcentre for the ID verification, do I send this with the ssmg form also? I don’t want to send original copy if I don’t have to, worried it’ll get lost! Thanks xxx
