My tired brain just realized you said areola and not nipple, in which case the silicone set ups may not actually help. You may need to size down more! You’ll have to purchase an insert to go smaller. But do try to get a consultation for proper fit. Last I checked they didn’t cost anything (from legendairy milk).

Sound like you have elastic nipples. I’d recommend the fluid fit shields from legendairy milk or the silicone flanges from pumpin pals. I know there’s another popular brand too but can’t remember the name. But a silicone setup and/or lubrication are gonna be your best bet. Go with the size you’re measuring at, in this case a 21. Also if you check the legendairy milk Instagram (probably the website too) they do consultations to help you get the right fit and they post videos all the time with wrong/corrected fits.
