Medicaid and primary care doctors

My daughters 1 month check up is on the 12th and they called to let me know that her insurance, which is simply health care I guess since that’s what I have, doesn’t have who she goes to as her primary care and they told me to call Medicaid but I looked up the number to call, they say I need an appointment but by that time I get an appointment it’ll be passed her 1 month or should I call my primary care that she’s supposedly under? Who I think isn’t even a pediatrician since I’ve never even been there. I’m so confused on what to do.
