Sick baby

So my house had a tummy bug and my little one has just been taking longer to get over it. He’s had a temp of 38.5 for day 4 now just can’t get it to break even with paracetamol and now it seems he has this little dots on his belly. I haven’t called 111 as we have been trying to deal with at home as usually it ends up been told to treat at home anyway, however I wanted to ask did anyone have spots on their little like this? I’m not sure what they are. Still having wet nappies and eating and drinking just less. But is very sleepy
