Hey, anyone here fill in the form on their own? I’m struggling to complete it, it’s taken me months! Some questions just seem super confusing or i simply don’t know how to answer them. Also what kind of paperwork did you provide if you are yet to receive a diagnosis?

You can provide, portage play plan and reports, SALT reports, health visitor reports, Pre school reports, sleep diary etc. What sort of questions are you struggling with?

I did, it took me a lot! I was given a quick run down but I’ve done it all myself and it’s super important to use certain phrases! My son doesn’t have diagnosis and we used Speech and language, any hospital/doctors records!

I was really lucky. I sat down with my son's SENCO and she did it for me lol I say there with a cup of tea.. im glad she did it cause I don't think I would of done it properly. She kept saying to me about basing it on his worse day but I'd play down alot of the answers cause I didn't like him sounding bad.. but obviously that's the stuff they need to know.
