Does anyone know if there is a form a father could sign to acknowledge mother as custodial parent? That could be filed in court without “going to court”. In other words notarized and filed.

I think it depends on the state you'd have to check with your family law advisor about court requirement. Custody has many levels depending on what your after - joint, living/ primary/ sole etc. Each may require certain proofs that it's best for a family lawyer to evaluate.
