MAT B 1 Form
So due to my circumstances i qualified for the sure start grant which requires me to send my physical MAT B 1 Form off. But work have asked for it also to keep so I was just wondering if anyone has experienced this, or have any advice as I don’t want work to take and keep the form as I need to apply for the government grant?

Your work only need a photo copy of this or a scanned copy as you’ll need the physical one for other things too (free dental care/ eye tests etc) xx
Your work place don’t need to keep it, so just ask them to photocopy it and keep the copy x
I have 2 jobs and only 1 form.. they just photocopied and gave it me back 😊
My work just photo copied it and I just photocopied it for the grant, you keep the original one (you only get one so don’t send it off)
