The box on the formula will tell you how to split the formula to water ratio so you don't need to worry about that. It also gives the age as a guideline.

I would start off at the lowest if breastfeeding doesn't work and if baby drains the bottle and still appears to be hungry move up to the next amount.

As for how often - as long as baby is gaining weight, isn't premature or have any other problems I would leave baby to sleep and let them wake you/tell you they are hungry. I found with my first it was every 3 hours ish as I did formula due to not being able to breastfeed. X

If you plan to breastfeed, please know that first days are key for milk supply buildup, and hand expression and frequent breast stimulation is key. The first 2 week can be around the clock breast stimulation but this help you body to build your milk supply.
