Silver nipple shields saved my nipples while breastfeeding. I got some off Amazon but think any version will do. Leave a drop of breast milk in them and have them in your bra. The healing properties in breast milk helped my nipples and kept them from rubbing and getting sore between feeds. They don’t look the best in your bra but fine while you’re at home. I would also think about using a Hakka on the right side while feeding on the left to try and stop engagement even if you don’t want to store the milk. Definitely speak to your HV for some advice ❤️ Second the above comment maybe try different positions too. My little girl also liked being fed laying down as I think she felt comfy and relaxed. My letdown was very fast so I sometimes found if I expressed a tiny bit off before a feed my little girl preferred it too.

Can you hand express to ease rhe engorgement? Or pump if that doesnt hurt? that's what I was recommended when I was engorged the first time. I would also try doing a different position, my right boob was excruciating at times (I did have mastitis once early on!) and I found that lying down to feed helped as the angle was different! Last thing would be to see if you can talk to your HV or get to a breastfeeding support group who know what they're on about better than me! It may even be thrush? But I don't know anything about that so would deffo recommend going to a group if you can for better support.
