Maternity exemption certificate

I'm 23 weeks. Still haven't received it. I feel like I'm constantly chasing my midwife for everything. Heaven forbid I forget I won't receive it until two months. If I get it, I can get free medication. How do you chase this up so I can receive it at the earliest? Will the antenatal clinic be of any help at all? It's really difficult to get to my midwife Thanks x

Mine was on my pregnancy notes under certificates if that’s helps, it was there from around 12 weeks I think

I can't find mine there.. It says enter certificate number?

I got a form to fill in and send off at my booking in appointment, it then arrived in the post. Perhaps like Beth said, check the pregnancy notes app and you may be able to do it online?

I may have misplaced it. I've just messaged my midwife to ask if she can do it. But she doesn't reply so I'm wondering if the GP would be of any help at all?

@Ceri-Ann I did use this link. But my details do not show up as it's not been registered. I've rung my GP and they had the form so I've filled it out.

Can ask your GP for a form to fill in and give bk
