Immunization records for school application

Hello, so we live in the Peel region (Ontario) and I didn't know we were supposed to upload our child's immunization records online. This generates a number that is required on the school admission form. I just started the process and using my child's immunization record (yellow card) and notice that the healthcare providers have not mentioned the vaccine brand name and that is a mandatory question in the form. Wondering if anyone else has been through this process and has advice? Thank you.

You don't need to fill out that part of the registration form for kindergarten. uploading the immunization record online is enough. I think the form was made before records needed to be uploaded and just haven't changed the form..my 4 year old still hasn't had his 4year old shot and we already filled out the registration forms months ago and left the immunization page completely blank ...I've also always left it blank for daycare and just have shown them my yellow record book and uploaded it. 🤷‍♀️

Yea I did the same for our daycare, sent them a photo of my yellow record book. The admission form requires a 9 digit number that I'll get only after I submit the immunization records online. However I noticed that our healthcare providers have not mentioned the brand name of the vaccine 🤦 will need to give them a call.
