She could possibly be in the wrong sized nappy

My boy does this every time we need to move up a size in diapers. Even if the weight has been within what it says in the box, I’ve always had to go up

I had to move up a size for my 3 week old for the same reason. Didn’t get much use out of the size one nappies. Was having to change clothes throughout the night

Check vests are not too small as well or they compress the nappy and cause leaks

Id say size up diapers if it still happens maybe change brands.
