Part time work request

Hello, this post is not strictly baby related but going back to work after maternity. I would like to make a part time work request at my work for when I am due back in April. I contacted them already letting them know about my intentions and received a form back that I need to fill in. I have no idea what to write in points 3 and 4 as I feel whatever I will say they will try and negate. My team has about 20 people in it and every month or so, someone is leaving amd a new person joins that requires training etc . There are other teams in the company where people work part time, I would be happy to go to another team etc, I would not require training if I move teams. Has anyone done something similar, what would you write in points 3 or 4? I feel they will decline anyway but I want to have the best shot. I would like to go back to working 3 days but would be OK with 4. Sorry for a long post. Appreciate any ideas from you.
