Protein/ketones in urine

Hi all diagnosed with GD and been on lower carbs for a few weeks. Slightly worried because they found ketones in my urine, they’ve sent it off to the lab just to check it’s not a UTI- but just worried. Anyone else had this with GD?

Yes it's called diabetic ketoacidosis. Need to be very careful about it.

Gp said to me it happens more when you try to cut carbs incorrectly.

Diabetic nurses told me to make sure I am having carbs but more.complex carbs so body takes longer to break it down

It can also happen if you are.on wrong insulin doses.

Make sure you are hydrated and drinking lots and it should help

I've been told if it happens to me again I need to go to hospital and get put on an IV

A&E/ED nurse here ... Not exactly diabetic ketoacidosis, to have this you need to have 3 things present ... Blood glucose levels over 11.0, 2+ ketones in urine and a blood pH of less than 7.35. You need a blood gas sample taken to be able to diagnose DKA. When we have people present to A&E/ED with confirmed DKA they are usually EXTREMELY ill and need a lot of treatment. Please try not to go down the DKA rabbit hole as it can be scary reading

But yeah, cutting carbs completely will produce ketones in your urine, just make sure the carbs you're eating are wholewheat/slower release and don't see them as the enemy, you still need them in your diet. I cut them out completely when I first got diagnosed with GDM and kept having high ketone levels in my urine, I've found a better balance now and only have small amounts present x

oh thanks for explaining that I was told it was DKA with me and I needed to attend a&e immediately but we managed to bring the ketones down quickly!

It's happend a couple of times since I was diagnosed with GD

thank you hun, so I haven’t been cutting out carbs per say and example is I’ll have a wholemeal pitta with a meal or a slice of wholemeal toast then x3 egg sized new potatoes. Does that sound ok carbs wise? I think the gestational diabetes.co.U.K. website says about 3 tbsp of wholemeal rice with a meal which I have also. Just feel at loss, I’m worried if I eat more my readings will go through the roof :( xx

trying, currently drinking 2.5-3 litres of water a day. Just started on metformin this eve. Just getting so frustrated by all of this and really down. :( xx

yeah I drink as much as I can I take 4 Metformin a day 12 units of slow release insulin overnight and between 3-5 units of rapid insulin during the day b4 each meal

I kept having severe ketone levels and all sorts so they gave me an in arm glucose monitor so I can help monitor my sugars. The ketones kept happening with a drastic sugar change for me.

Mostly I've switched to wholemeal carbs and halved the carb portion.

I will say I've been stable for a week with that now. Sugars are closer to what they should be!

Diabetic ketoacidosis is only one of three types of ketosis that could be happening, and the level of ketones in your urine isn't actually a good indicator of the level of ketones in your blood. Diabetic ketoacidosis IS bad and can be diagnosed with a blood test, but ketones in urine can be high from nutritional ketosis (more likely if you've been on a low carb diet) without it meaning anything bad at all. Some doctors associate all ketosis with diabetic ketoacidosis which is not correct and can make things confusing. One of the later chapters in Lily Nichols' book Real Food for Gestational Diabetes talk about this.
