Anyone know what this could be?? I’ve had it for a couple months now and I’ve been putting hydrocortisone on it and it helps but the second I stop using it it comes right back.

Personally I’d stop using steroid cream. It thins the skin and your skin goes into topical steroid withdrawal and worsens when you stop using. The fallout from that can scar.
I’ve had super bad perioral dermatitis before and after seeing a dermatologist it healed really well with a combination of non steroid creams, and it never returned. Took a month but I don’t have any marks or scars. Get seen cause it could be something easily fixed if it’s hormonal.

You can try anti-fungal cream instead of cortisone- try twice daily for a week

Steroid cream longterm is harmful for the skin. Hopefully the doc can give you a different medicine or a guided plan on how to use a steroid cream .
