Visual Birth Plans to Share?

Before I start making a visual birth plan, I was wondering if anyone would be willing to share theirs with me if it is editable? I am looking for things like intermittent monitoring, no Pitocin, no epidural, no pulling on chord after delivery, delay chord clamping until pulsing stops, no jabs, no erythromycin.
