Return to work

I am nowhere near ready to go back to work but i am due back in 2 weeks.
I want to put in a sick note but I dont know what to say to the doctor!
My baby is not sleeping well (awake every 1-2 hours) nearly every day I nao with him to get through the day. I couldnt manage work.
What should I ask to he put on the sick line?

I have worked in this post for 17 years and have been sick twice. I feel like i should mention that because I dont normally do it
