Maternity allowance

The day you go on maternity leave is when you want them to start paying you MA, surely anyway

I can’t help with the form, but I was in a similar position with work saying they wouldn’t pay SMP. I questioned it and they insisted I didn’t qualify. They then backtracked and said I did, even though they know I won’t be going back to them after maternity (end of fixed term contract). So it might be worth asking to speak to the senior HR person or whatever.

that's why I'm questioning it myself because if I look at my payslips within the test period I should be easily qualified for the smp

Call HMRC and find out if you do qualify before you send it off as they will be able to tell you what you should be receiving

I put the date I started maternity leave, and got my first payment yesterday so must’ve done it right!

You put down the day your maternity leave started. As that's when you want it paying from. If you need any help message me as I had to fill in the same form

Reading this I may have done mine incorrect 😩🤦‍♀️ I only work weds- Fri and finished on a so put down the following Wednesday (my next day in work) down for starting my MA

My work informed me they won’t be paying smp so I’ve just applied for this…
How long they take to get back to you? As I am already a month behind on my pay xxx
