My six week old is sleeping well (for a newborn!) but really struggles with discomfort when put on his back. He arches his back and squirms a lot after feeds anyway, but is most comfortable in an upright / inclined position. Even after staying upright 20 mins (and we’ve tried lengthening this right up to an hour!) when lay down the back arching becomes significantly worse and he’s gulping and gasping until he gets so distressed and it escalates to wailing.
He can’t tolerate being on back more than 10 mins or so when awake either (pram walks, changing mat etc).
As soon as he’s put back in an upright position he’s fine and sleeping away again on me. It seems more than just comfort / contact needed, even co sleeping he won’t lie down next to me.
How on earth do I balance safe sleep guidelines, with making sure he’s getting sleep and then also ensuring I am able to rest?! It’s meant for weeks now my partner and I take turns having him on us almost 24hrs a day and I’m running off 3-4 hours sleep per day even though our baby is sleeping much much more than this but I have to stay awake as he’s on me / upright.

It will probably be reflux. See a cranial osteopath- that helped us significantly. We also went to GP and tried gaviscon (made her constipated so stopped it straight away) and then omeprazole which really helped ( lots of side affects to omeprazole so consider) we were on omeprazole for 3 weeks before we stopped after reflux improving.
Before that we used to do chest cosleeping (happy co sleeper on Instagram helped) or I would lay her next to me with my arm under her neck / head to elevate
When she was in her crib I placed a tiny pillow under the mattress to tilt it to an angle.
In the pram I put a curved tummy time cushion in to keep her elevated

I’d also invest in a good baby carrier. Game changer for allowing you to get stuff done too m

Sounds like reflux, our little one has had it since 1.5 weeks old, so literally sounds the exact same thing! Although it's possibly silent reflux if your little one isn't sick as well.
Doctor gave us Gaviscon, which did help, but not enough, then advised we picked up Carabel from the chemist - wow - quite a big difference, we've ditched the Gaviscon at the moment and are just using that - definitely better than Gaviscon!
I'd give it a go, nothing to lose - you don't need a prescription, bit you do need to get it from the chemist, nowhere else seems to do it x
