I would ask. Apart from anything just so they know it wasn’t done at home. Not sure where you are based but in the uk we get an accident form. Every time my daughter bumped her head with her best friend we would get a slip of paper.

I've done this. I've messaged the nursery on 2 different occasions when my boys come home with bruises/bumps that haven't been mentioned so I don't think it's an issue you messaging this

Definitely mention it, I'd email so there's a paper trail of this. If they saw him fall he should have been checked over to make sure he was ok
