How old is he?

Does he look uncomfortable or does he complain about hurting him? Maybe second opinion from different doctor, if you could get one.

Could be smegma like when his privates are cleaned enough and the oil, skin, bacteria gets built up. Go to pediatrician and they can help. Wash it thoroughly

he is two years old. He is fine. He is playing and playing it just looks horrible and when I change his diaper he complains but it might hurt when I wipe.i put an ice pack wrapped with a wash cloth on him and it seemed to help he is also taking ibuprofen to help. It just looks horrible

I have 3 boys and one thing I always always checked for was a small hair wrapped around any part of it which will cause a tourniquet and swollen and bruised red purple if not caught. Check balls and Pp everytime just in case. Kinda like the toes how they catch hair, same with boys and their penis it just wraps around. I have caught a few hairs already and I always get so worried. Other than that maybe he fell or pulled it or a dog if you have one scratched is my only other guess. Hope he will be okay! Coconut oil helps with inflammation too- good luck and big hugs

Is he circumcised? If not they can get bacteria and infections in it.

What about bath with Epson salt?

I think you should take him to the urgent care and let them assess him because that could be anything. Also sounds like an infection

Oatmeal baths, warm water, diaper balm to help with itching once swelling comes down. My son had a uti (circumcised for those who like to talk crap…) and had swelling since it was foreign to him… maybe also go in for a second opinion. Did they do a urine culture? Bloodwork?

Sounds like he may be uncircumcised and has an infection. Very gently see if you can get any pus to come out by very gently pulling the skin back (don't force it farther than it will naturally go) just a little and then from the base of the shaft, squeeze a little as you move towards the head of the penis. If you can get pus out, try to get as much as you can without it becoming torturous because this can be quite painful. Then, pull the skin back again (without forcing it farther than it will naturally go.) Wash his penis with unscented soap, twice a day until this clears up. Pat it dry. Use a q-tip to whip a little Vaseline in the container of regular Vaseline and then put a thick layer all over the tip of his penis. This will protect it from moisture. It's important to whip the Vaseline because it makes it so much easier to get enough on the q-tip and apply it. Do this twice a day until it clears up and make sure you are pulling the skin back (without forcing it) to clean during every diaper change/bath.
