For context it's my MIL, and she is quite well off, can buy whatever she needs as she needs it. It's so hard to buy anything for her!

Maybe something personalized?
Idk the occasion but maybe framed photos or a small photo album, maybe an art piece made by you or by the kids, or a paint-your-own ceramic item that she could use or display? When I'm gifting someone that can buy what they want I try to get something money can't buy and that will remind them of the people who love them.

Depending on babies age a painting they’ve done or footprint art.
Then maybe something she may not get herself? Like something on Groupon or wowcher. Like afternoon tea, spa day, visiting a sight, steam train etc.
or a necklace with a photo of you all on it (you can get ones that look normal and then will protect an image on the wall).
Anything she likes? Self care, sports, eating out, theatre, cinema?

Something sentimental or handmade, homemade coffee syrup in a cute little mason jar with a ribbon, a book, or if you really like her maybe plan a little outing for her and the grandkids like a picnic, tell her to dress kinda cute and take some pictures of her and the grandbabies. Or you could get her one of those “I want to hear your story” type of journals for her to answer questions in there and then it becomes a keepsake for the grandkids to always have, you know when she’s gone 🫠 maybe that’s kinda morbid tho nvm

Something sentimental like a photo book of her family. Or a family day out.

Dont buy anything make her something. Because money cant buy something that comes from the heart

A day out. For her (with or without you) surely she couldn’t have visited that one place you can find- restaurant/ experience etc
Or a day out with her grandchild (if you like her to go together lol)
