Are the bottles being put together properly, new born nipple or extra slow flow, he might not be getting a good latch… if worse comes to it try spoon or syringe feed him

@Aspen his latch seems grand, you can really feel the tension if you try pull the bottle away and you can gear the suction releasing when he unlatches, bottles are put together properly but haven’t tried a test any smaller than size 1 or 0-2mths, might have to look into that!

It’s not the bottle I think, it’s him. Has he been checked for tongue tie? Messy eating is one of the signs. My girl used to be like this, she was checked properly at 6 weeks old by a lactation specialist, and turned out she’s had posterior tongue tie. After it was corrected it’s got better, she still has a bit of milk around her mouth though every time she eats… she’s 7.5 months now. We used Tommee Tippee bottles for a long time and they never leaked for us. We are using Avent Natural now, we have 6 of them and had issues with one bottle that kept leaking if the lid wasn’t on properly.
@Boglárka I was starting to wonder about a tongue tie tbh, do you just go through your gp to sort this? I know he’s still getting milk as hes gaining weight really well but regardless, it must be uncomfortable being drenched in milk:(
I have the same issue, I think it’s the mouth being filled with too much milk so it just spills out. I used size 0 teats with my mam bottles until he seemed to get bored of the slow flow 😂 but to be honest it never really solved the leakage situation… baby also spits up a lot so now I’ve just accepted that I need several outfits a day! I do also use a bib/muslin tucked under chin to catch most of it so I don’t always need to change him.

You have to slow him down when he eats. My little was having the same issue. My pediatrician recommended me to take the bottle out of their mouth every half oz. Make sure to break their latch with your pinky when you do though. Once I started doing that my little has done great and we haven’t had so many issues.
@Brittany what position are you holding to feed? I don’t have a spare hand to break the latch with my pinky as I’m holding bottle in right hand and holding him up with my left

@Aspen agreed! He may need a slower flow nipple. You can also watch videos on YouTube on "pacefeeding" usually when there is leaking or pooling of milk it suggests the milk flow is too fast for the baby to manage. Pace feeding is essentially baby held up right 90 degrees, bottle tipped horizontally ( just enough so the nipple stays full) and breaking babe If you see pooling or hear gulping sounds.
@Rabeka usually a cradle hold with my left and the bottle with the right. I gave MAM bottles so it’s small enough for me to just slide my right hand down the bottle to use that pinky. It took some getting used to at first but eventually I got the hang of it.
If it's an anti colic bottle make sure the little air hole is pointed up when you feed him, also make sure that it isnt bent outwards if you clean it with a straw. Idk if that makes any sense but it's the best way I can word it.

It definitely sounds like a size 1 teat is far too fast for him, I’d suggest trying a size 0 teat to see if that fixes the milk spilling out. It doesn’t sound like a latching issue especially if there is tension when you pull the bottle away, and he’s gaining weight.
Look into pace feeding and burp
Try a preemie nipple! Even if he isn’t a peeemie. I recommend the Dr. Brown’s preemie
It could be a tongue tie issue. My LO was also leaking milk and it was the issue with his suction because of posterior tongue tie. Como tomo bottles seem to worked better than others
Try a slower flow nipple (size 0 or premie). But him leaking some milk can be normal, as long as he’s making plenty of wet diapers and gaining weight well then he is getting enough!
Primark do a dribble proof bib which has a plastic lining which could help protect his clothing whilst you look into tongue tie/different teats/feeding positions etc x
I used Avent Philips bottles for both my babies. It is the closest to breaststroke for breastfed babies. It does not let out any milk unless baby is sucking on it. It's not a slow leak like other bottles. They literally have to suck hard on it to get milk, similar to if they breastfed. The other one I used for 6m and up was the Boon Nush bottles. But it's because it had a slow leak that I did 6m and up. With avent Philips, you can switch to more flow as they get older by buying the nipple sizes for each stage newborn 0+ flow, 3m is more flow etc.
we use doctor browns and allow our daughter to feed facing the side so it’s more at her pace and less forced - i don’t know if you’ve tried either of those things but worth a shot!
how often are you feeding 4 oz?
You may need a smaller sized teat, less milk will come out of the bottle. I had the same problem with my little girl, especially when she goes feral for the bottle 😅
My daughter was the same, per our pediatrician’s recommendation we switch to Dr brown size zero anti colic and she did great afterwards. Some babies have such strong latch, which is a good thing 😊
I’d swap to the slowest teats you can get and ensure you are properly pace feeding. Make sure baby is upright when feeding. Not lying down and don’t tip the bottle all the way up. Dr Browns are quite slow paced bottles, particularly compared to MAM.
Slower flow nipple or I add cereal or oatmeal to my daughter's bottles. She chugs like she's starving lol

I would have him checked for tongue tie too. My son was the same but it all improved after treatment

I’d get a check for tongue tie - but use slower flow teats like someone said. Also - 4oz? That’s a lot for 5 weeks old! My 3m old is only on 4oz… maybe try smaller feeds but more frequent?

Be careful about not over tight the lid, try the bottle/ nipple the is closer to the real thing.
Recommend phillips avent natural flow ones. It doesn't let milk out unless baby is sucking so you wouldn't have the overflow problem
Are you pace feeding?
Pace feed, mam bottle with size 0 teat. Is he clicking on the bottle? Definitely research into signs of tongue tie, my friends son had 100% tongue tie and he had milk coming from the sides of his mouth during feeds x
My daughter did the same thing when she was a newborn until we found out she needed a slower flow nipple!!
I have a similar issue with my little girl but there’s no tongue tie issue or anything like that but she gets frustrated with the 1 flow teat so I have to give her the 2 flow and just pray the bib catches it but she has started to not spill as much over the last few days, she’s nearly 4 months x
Are you using wide neck teats or narrow neck teats? We tested out Philips Avent Natural bottles which use wide neck teats and had this exact problem. So we switched to Philips Avent Anti Colic bottles which come with narrow neck teats and that solved the problem for us :)

@Rabeka we’ve found a private one, but turned out the same person does a feeding support group in our area. Try to look up local breastfeeding support if you don’t want to go down the private route. I also agree with other commenters here, maybe worth trying a slower tear first.
@Rainé clicking during feeding is a break of a seal which is usually caused by an oral restriction (i.e tongue tie). The break of a seal allows air intake and causes reflux, so would recommend getting a private oral assessment by a qualified tongue tie specialist
