Could someone please clarify the gov make these things so confusing and my post partum brain can’t take it!
I believe I have to return to work on 1st May (potentially using annual leave) or I am not entitled to my babies 30 free hours until sept. Is this because the last 3 months of maternity fall under the pay threshold even if you have chosen to have your income spread equally across 12 months?
My other question is would this affect the free hours I get for my eldest? If I don’t go back on 1st will they lose their hours? Or what happens if the reconfiguration period falls within this last 3 months of maternity to I lose that entitlement?

No, it's not because of the pay threshold. It's because you need to be back to work by 30th Apr (not 1st May!) to qualify. It's purely about dates. I'm cutting my mat leave short by 1 week and going back on the 30th Apr precisely for this reason.
And no, it doesn't affect your eldest's entitlement in any way xx
PS. You can be on AL from 30th Apr and you will qualify. I'm not really going back to work until end of June because I'm using a year's worth of AL after mat leave.
