My community midwives said it would be via them if I ever wanted to know they could do a debrief type thing

I did! I got told that every hospital has a team who deal with these so you need to find that team and send them an email. Give them details of yours and baby name and date of birth and address and the time you were in their care and hospital, hospital ward and gp you were attending in the request email.
This should avoid them trying to delay them sending them to you and jf they do do that you can fight against it because you sent all ure details x

Yes everyone is entitled to do a Subject Access to Records request for all areas of healthcare.
The NHS trust that the hospital is in will have either an online form to complete or an email address to contact. If you Google SAR and the hospital it should tell you how.
I got mine back within a month and it was every note from my booking appointment at 10 weeks through to discharge from postnatal ward.

And it doesn't matter how long, they have to hold records for years. I made my request I think about 16 months after my son was born

I requested my full maternity notes via a subject access request. https://www.nlg.nhs.uk/patients/additional-support/health-records/ You can request an application form by emailing the address on this page

Contact PALS, you will get a form to complete and they probably require your ID. I requested information from the hospital I started at before being transferred to another hospital before baby was born.

I had a birth review which was really helpful as they go through the whole lot of notes with you and explain what everything means in plain english not doctor talk. I didn't get my note from that but I did have them explained. I think she did say at the end that i could request to have them if I wanted them.

Can someone tell me how I get them?? My labour was 2025?
