I’m self employed so may be different but I think you get a form around the 20 week scan time to fill in and submit?

I didn't receive anything. I'm an employer. I'm 22 weeks now. I've already done the 20-week ultrasound. But I'll have an appointment with the midwife next month. Does it have to be the midwife to give me something to be able to apply?

ask for a mat b 1 form and they'll do their bit of it then you fill it out with the date you want to start leave on and then just give it to your employer

Oooh odd! I think it’s a certificate confirming the pregnancy/due date! Think it’s called MATB1 and you get it from your midwife?

So I think if you’re self employed for over 26 weeks before being pregnant you need to fill in a MA1 form which you get off of gov.uk and send a copy of your certificate but if you’re employed then you need to tell your employer when you want to start your Mat leave and show them the certificate!

I’m not 100% sure but that’s what I’ve been told! I’ve been SE for about 7 years now so I’m only allowed Mat Allowance instead of Mat Pay x

It’s meant to be from 20w appointment that you get your MATB1 and you have to tell your employer no later than 15 weeks before your due date so basically at 25w latest, so if your 22w rn you’ll have about 3 weeks to get it and send over to them. you can ask your midwife, or if they’re pretty terrible, ask your GP if they can provide one!! let me know if you wanna know anything else x
