Hey moms! I'm hoping someone here can answer my question because finding the answer online has been very confusing.

My leave wasn't paid for by my employer. It was California state Short-term Disability Insurance benefits

I am in nj and the form I needed for my taxes was in the online portal where I filed the claim. They didn’t mail me anything
