It needs a stamp from your GP surgery so it's a paper copy your midwife should give you

You have to make an app with your midwife she will give you a copy x

You will be given a physical copy by a midwife. Your employee will require the original form. If you dont your maternity pay will not start. Or like me theyll continue paying you your normal wage and then you owe them money back when you return (yeah that was a headache to sort out when ym daughter was nearly 2 months old!!)
